A Personalised Aged Care Plan, Built Around You

Every older person’s situation is different, so a plan pulled off a shelf rarely fits. A personalised aged care plan is a roadmap built around one person’s needs, preferences, budget and wishes, and updated as things change.

Together we work out what good looks like for you and your family, then set out the practical steps to get there. It might mean care at home, social support, assistive technology or home modifications, or planning ahead for a future move. Your plan isn’t set in stone. We revisit and adjust it as needs shift.

How we build your plan

01
We listen

A conversation about the person, the family, the worries and the wishes, not a form.

02
We map the options

We lay out what’s realistic, what it involves, and what it may cost.

03
We write it down

You get a clear plan you can act on and share with family.

04
We stay in touch

As needs change, after a hospital stay, a diagnosis or a change to aged care rules, we adjust the plan with you.

One plan, one person to call.

The hardest part of aged care isn’t any single decision. It’s holding all of them together. Your plan keeps everything in one place, and we stay alongside you as your guide.

What is a personalised aged care plan?

It’s a written roadmap built around one person’s care needs, budget, living preferences and wishes. It sets out what to do now and what to plan for later, and it’s reviewed as circumstances change.

Your aged care assessment produces a support plan based on your assessed needs and the services you’re approved for. Ours is broader and independent. It weighs up home care, private options, retirement living, residential care and future costs together, and we help you act on it.

No. We explain funding and costs in general terms so you understand your options. For personal financial or legal decisions, we encourage you to talk with your existing financial adviser or solicitor. If you don’t have one, we can introduce you to someone suitable, on merit and with no fee in either direction.

Yes, that’s the point. Care needs rarely stand still. We revisit and adjust the plan after key changes like a hospital stay, a new diagnosis, or a change to aged care rules.

That’s up to you. We can guide you to compare providers and understand the fees so you choose, or we can manage the whole process for you, including finding and securing a place in the right home. Because we take no commissions, our recommendations stay independent.

Why choose us

Why older people and families choose Aged Care Conversations

Our guidance comes from people who have worked inside the system and still do. Louise Greene is a Registered Nurse with an MBA and more than 40 years in health and aged care across clinical, executive, consulting and board roles, including chairing clinical governance committees. Kerry Mann is a former CEO in aged care and retirement living, holds an MBA, and has board experience including Ageing Australia’s NSW/ACT State Member Council.

How much we do is up to you. Some people want a single conversation to get their bearings. Others want us to manage the whole process, from the assessment through to finding and securing a place in the right home, with regular updates along the way.

We’re independent. Older people and families pay us directly, and we take no commissions from aged care homes, retirement villages or providers. When we help with placement, we’re working for you: homes pay us nothing. Where we introduce a partner, it’s on merit, with no fee in either direction. People come to us in the hardest weeks and leave with a clear plan and a person to call.
