Ongoing Aged Care Guidance for the Whole Journey
Life doesn’t stand still, and neither do care needs. The right support today may not be the right support in six months. We stay alongside you as things change, so you always have a trusted guide, not just a one-off answer.
Whether it’s moving from respite to permanent care, adjusting after a hospital stay, or making sense of a change to aged care rules, we’re here for the long haul.
When ongoing support matters most
When someone comes home needing more help, or can’t return home at all.
A new diagnosis, a fall, or a gradual decline that shifts what’s needed.
Turning a short stay into the right long-term choice.
Changes such as the Aged Care Act 2024 and Support at Home affect real people, and we keep you current.
Arranging respite before burnout sets in, and connecting carers with support such as Carer Gateway, the government’s free carer support service.
What ongoing guidance looks like
A call when something shifts, not a form to fill in.
Update the roadmap to match the new reality, including a reassessment through My Aged Care if needs have changed.
New services, a provider change or a move, with us guiding you or managing it for you.
Rule and fee changes explained in plain English.
People tell us the biggest relief isn’t any single answer. It’s knowing there’s someone who understands their situation and picks up the phone when things change.
Independent, and in your corner
As an independent service, our loyalty is to you and no one else. We’re not tied to a provider, so if a service isn’t working or a better option appears, we’ll say so. That’s what being in your corner means.
Frequently Asked Questions
What is ongoing aged care support?
It’s continuing guidance as care needs change over time: checking in, updating the plan and coordinating the next step, rather than a single one-off answer. It means you always have a trusted person to call.
Can you help after a hospital stay?
Yes. Coming home from hospital often changes what someone needs. We help you arrange extra support at home, including short-term options such as Transition Care, which is arranged by the hospital before discharge. If home is no longer safe, we work through the options calmly, including residential care.
Are you an aged care advocate?
We act as an independent guide in your corner. For formal advocacy, the Older Persons Advocacy Network (OPAN) offers free, government-funded advocacy on 1800 700 600. Complaints about a provider can go to the Aged Care Quality and Safety Commission on 1800 951 822. We’ll point you to either when it fits.
How do you keep up with aged care reforms?
Staying current is core to what we do. We’re both active in the sector, so when rules change, as they did with the Aged Care Act 2024 and the Support at Home program, we translate what it means for you in plain English, so you’re never caught out.
Do we have to commit to ongoing support?
No. Many people start with a single conversation and come back whenever something changes. You choose how much support you want, and there’s no lock-in.
Is your guidance really independent?
Yes. Older people and families pay us directly and we take no commissions from providers, so our guidance always serves you, including telling you when something isn’t working.
Why older people and families choose Aged Care Conversations
Our guidance comes from people who have worked inside the system and still do. Louise Greene is a Registered Nurse with an MBA and more than 40 years in health and aged care across clinical, executive, consulting and board roles, including chairing clinical governance committees. Kerry Mann is a former CEO in aged care and retirement living, holds an MBA, and has board experience including Ageing Australia’s NSW/ACT State Member Council.
How much we do is up to you. Some people want a single conversation to get their bearings. Others want us to manage the whole process, from the assessment through to finding and securing a place in the right home, with regular updates along the way.
We’re independent. Older people and families pay us directly, and we take no commissions from aged care homes, retirement villages or providers. When we help with placement, we’re working for you: homes pay us nothing. Where we introduce a partner, it’s on merit, with no fee in either direction. People come to us in the hardest weeks and leave with a clear plan and a person to call.
